12:00 -12:30 p.m. Healthcare Quality Association on Accreditation (HQAA)	Art Woo, Pharm D, Infusion Surveyor, HQAA	Join HQAA Pharmacy Surveyor Art Woo for an overview of HQAA’s web-based accreditation application and preparation process. Learn about the online “work-room” method HQAA utilizes to review the applicant’s policies and procedures, and the benefits of having a dedicated “Coach” who works individually with each organization to simplify the complex process of accreditation.  Review the standards that guide HQAA’s pharmacy accreditation survey.
1:00 -1:30 p.m. Accreditation Commission for Health Care, Inc. (ACHC)	Kim Heagy, RPH, Pharmacy Surveyor, ACHC, and Matthew Hughes, Accreditation & Business Relations Manager, ACHC	ACHC’s Accreditation and Business Relations Manager Matt Hughes and Surveyor Kim Heagy will provide a review of the ACHC accreditation process, including a broad overview of the most critical standards that apply to home infusion pharmacy and nursing services. Learn about ACHC’s online resources and accreditation tools.
2:00 - 2:30 p.m. Community Health Accreditation Program (CHAP)	Linda Scott, MSHA, RN, COO, CHAP	CHAP has more than 40 years of experience accrediting alternate-site providers. Linda Scott will provide a comprehensive overview of CHAP’s home infusion accreditation services, including the online process for submission and review of organizational self-studies. Learn about their web-based “Plan of Correction” tool for addressing and correcting deficiencies identified during the survey.

1:30 - 2:00 p.m. Utilization Review Accreditation Commission (URAC)	Christine G Leyden, RN, MSN, Senior Vice President and GM Client Services, Chief Accreditation Officer, URAC	URAC accreditation for Specialty Pharmacy and/or Drug Therapy Management is a collaborative and educational approach to accreditation that recognizes the complex and interdisciplinary services being provided.  In this session, Christine G. Leyden, Chief Accreditation Officer with URAC, will give a brief overview of the organization, the accreditation process, and Specialty Pharmacy and Drug Therapy Management standards.
